What Should I Bring to My Orthopedic Appointment?

Medical

  • X-rays, MRIs or other imaging films
  • Information about what treatments and medications you have tried in the past
  • Any medical records that you think might be helpful
  • Questions you want to ask your physician

Financial

  • Current photo identification
  • Insurance information and card
  • Co-pays and any outstanding balances

Only if you haven’t completed Pre-Check

  • Current photo identification

New Patient or Second Opinion

  • Outside X-rays, MRIs or other imaging films related to the visit
  • Information about what treatments and medications you have tried in the past

What Happens at My First Orthopedic Appointment?

On your first visit, your physician will:

  • Review your records, health history and previous tests.
  • Discuss your symptoms with you.
  • Determine if further diagnostic testing is necessary.
  • If diagnostic testing is necessary you may be asked to schedule a follow-up appointment with the doctor to allow time for the test and evaluation of the test before a treatment plan is discussed.
  • Finally, your physician will discuss your options for treatment or prevention.

Depending upon which clinic or specialty service center your appointment is scheduled at, you can visit our locations page for directions and parking details or call (651) 968-5200 for directions.

Parking varies at each location. Some locations have free parking lots, while others have hourly parking ramps. Please prepare accordingly.

How Do I Cancel or Reschedule an Appointment?

Need to cancel? Please provide 24-hours advance notice. This will allow us to reallocate that time to someone in need of treatment. Call or text (651) 968-5201 or complete the web form to cancel and reschedule your appointment.

Good Faith Estimates

  • You have the right to receive a good faith estimate for the the total expected cost of any nonemergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
