These physicals are not intended to replace your annual primary care visits. If you are currently being seen by another provider for a preexisting condition or injury, we encourage you to discuss sports participation with that provider.
Please fill out the Physical Form below and bring it to the exam to save you some time. We will have them there as well to fill out on-site.
If you pre-registered, please arrive at any time during your half hour time slot. Walk ins are welcome! You can expect the exam to take approximately 30 minutes depending on the volume of family that are arriving at the same time.
If you are an East Ridge, Woodbury, Park, Mounds View, Lakeville South, Lakeville North, or Irondale athlete, your school’s certified athletic trainers will be on-site to meet you and answer any questions you might have.
Please note: After the exams, each student athlete’s physical will be sent back with the participant to turn in to their school’s activities office. Summit Orthopedics does not keep copies of the exams on file.
