Employer Paperwork

You are now less than 1 month away from your upcoming surgery. 

If you are currently working and your employer is requesting paperwork that needs to be completed by your surgeon, you can mail the documents to:

Summit Orthopedics, 710 Commerce Drive, Suite #200, Woodbury, MN 55125

or

Fax them to 651-968-5903.  Please address them to ATTN: “Dr. [Surgeon]’s Team.” 

Make sure to complete your portion of the paperwork before sending it in, and make sure to send it to us as soon as possible, since it can take up to 10 business days for processing. Do not bring this paperwork with you on the day of your surgery; it must be completed ahead of time.
