Pre-Op Physical

Spine Camp: Please call to schedule your class if you have not already done so.

Physical Exam: Timing Matters, do not forget to schedule your preoperative physical exam between 14 and 30 days before your surgery. Ensuring you are in good health before surgery is crucial.

GLP Medications: Please make sure you discuss discontinuing any GLP medications (examples: Ozempic, Wegovy, Trulicity, Bydureon, Mounjaro, Byetta, Victoza etc.) with your provider at this appointment.

Employer Paperwork: If your employer requires forms completed by your surgeon, submit them (after completing your portion) to Summit Orthopedics via mail (710 Commerce Dr, Suite 200, Woodbury, MN 55125) or fax (651-968-5903), ATTN: Dr. [Surgeon]’s Team; allow up to 10 days for processing.
