What’s a health care directive, and do I need one?

A health care directive gives your family direction on what medical care you want or don’t want if you cannot communicate. It is suggested for everyone—whether or not they are having surgery.

If you would like a health care directive (living will), and do not have one, please talk to one of the surgery center staff and they can supply you with a form and give more information. Or we have conveniently linked to the current forms for both Minnesota and Wisconsin below.
